Event Medal Order Form

Medal Ordering Eligibility Criteria

Each student who earns a Superior rating in Solo & Ensemble is eligible to receive an AVA Solo & Ensemble medal for this event.
Each student participating in a choir that earns a composite superior rating at State Choral Performance Assessment is eligible to receive an AVA State medal for this event (the sum of 4-6 points, except with a 3 in sight reading).
Only directors are authorized to purchase medals.
Medals may be ordered for Superior ratings earned in previous years, provided that medals have not been previously purchased.
Orders for medals will be processed as you submit this form, but will not be mailed until a check is received for the total due.
Shipping and handling are covered in the price of each medal.
To ensure orders will be received by the end of the school year, orders must be placed and checks received before May 1st.

Upon submitting your registration, you will immediately receive a confirmation and invoice by email. If you do not receive the confirmation/invoice after your submit your registration, please let Matthew Gardner (matthew.gardner.ava@gmail.com) know immediately.
